CDC/CRMLNによる国際脂質標準化

CDC/CRMLNによる国際脂質標準化事業は、2012年3月31日まで大阪府立健康科学センターが運営していましたが、2012年4月1日から、国立循環器病研究センターが運営しています。(http://hospital.ncvc.go.jp/section/prevent-examination/)なお、この事業は、国立循環器病研究センター、大阪大学大学院医学系研究科社会環境医学講座、大阪がん循環器病予防センターの三機関の共同研究事業と位置付けて推進されています。

米国の国立医学研究機関であるCDC(疾病対策予防センター)は、インフルエンザや伝染病などの感染症に対する世界的な予防対策基地として良く知られていますが、内臓脂肪症候群(メタボリック・シンドローム)などの生活習慣病の危険因子とされる脂質(総コレステロール、HDLコレステロール[別名:善玉コレステロ-ル]、LDLコレステロール[別名:悪玉コレステロール]、トリグリセライド[中性脂肪])などの測定値を標準化する国際拠点として、50年も前から知られています。このことから、CDCはWHO(世界保健機関)の協力センターに 指定されています。標準化とは、病院などの検査室が正確な測定値を受診者に報告できるように支援する精度管理システムのことです。

日本内科学会は、メタボリックシンドロームの診断基準として、次のように規定しています。

  1. 内臓脂肪(腹腔内脂肪)の蓄積がウエスト周囲径 男性≧85cm 女性≧90cm
    (この項目に加えて、以下の2項目以上が該当したときに、メタボリックシンドロームと診断されます)
  2. 高トリグリセライド血症 ≧ 150mg/dL
    かつ/または 低HDLコレステロール血症 < 40mg/dL
  3. 空腹時高血糖 ≧ 110mg/dL
  4. 収縮期血圧 ≧ 130mmHg かつ/または 拡張期血圧 ≧ 85mmHg

メタボリック・シンドロームを正しく診断してもらうためには、正確な測定値に基づいて行うことが重要です。そのためには、精度管理システムの一つである検査室の標準化は避けて通れません。

CDCを中心とするCholesterol Reference Method Laboratory Network(CRMLN)は、世界7ヶ国から成る8施設の脂質基準分析室(下の地図を参照して下さい)で構成される国際ネットワークのことです。このネットワークの一員として1992年7月から2012年3月まで大阪府立健康科学センターが正式登録されて事業を展開してきました。なお、この脂質基準分析室は2012年4月から国立循環器病研究センターへ移管されています。わが国はもとより、アジアでは最初の脂質基準分析室が、世界中の臨床検査室や試薬メーカーを対象とした脂質標準化で活動してきました。CDCやCRMLNの基準分析室で求められた高精度の目標値は、各国の基礎的な調査(例えば、わが国では、毎年11月に厚生労働省が実施している国民健康・栄養調査など)、大規模な疫学研究や臨床試験(治験)等で得られた測定成績の信頼性を保証し、相互比較するための基準として、欧米諸国で広<受け入れられ、今日では世界的に評価が定着しております。

標準化は、皆様の目に直接触れることのない舞台裏の地味な仕事であります。このような信頼性の裏付けとなるトレーサビリティー・システム(標準化)があってはじめて、府民の皆様に質の高い測定成績が医師を通じて返却されております。
